If he's being asked to head the ball too frequently - which isn't exactly his specialty - it could affect his ear and cause an infection. Especially if the ball hits him on the ear directly.

I know you're smiling because of my clear dislike of Buttler in the test team but when the bowler, first and second slip go up for an appeal but your wicket keeper puffs his cheeks thinking the ball was close to the bat when it actually took a thin edge there's something wrong.

Honestly he's incredible. Smith, Williamson, Kohli only provide with the bat and in the field sometimes and then Cummings, Rabada and Bumrah with the ball. Holder is currently ranked as the best all rounder in the world and is arguably better than Stokes with the ball but his batting is far weaker than Stokes' in comparison with how good they both are as bowlers.

For everything that he brings for me he's the best player in the world

Absolutely. As an all rounder he really is the best - and I know the rankings don`t have him as that - but he doesn`t really have a weak part of the game. As you say, there are others in the conversation but they don`t have the complete all round abilities of Stokes. People forget that fielding is such a big part of the game and there aren`t many better than him either.
