You need them to finish in the same position in their respective groups. IE if they both win the group they will avoid each other until the final. But if Germany finish second and Belgium win the group they meet each other in the quarters.

this a standard yellow card with Sky Bet or has the ref got to show it in his left hand for it to count?

Backed no goals and no cards at 100/1.

Martinez has already said they don't want to finish top, so as it stands England will having accumulated one fewer booking. Think Southgate is pretty professional and won't instruct the players to pick up cards despite going to the harder side of the knockouts. If there's no cards after 70+ minutes could always back the overs as cover. Match could play out as a glorified friendly.
