If he's being asked to head the ball too frequently - which isn't exactly his specialty - it could affect his ear and cause an infection. Especially if the ball hits him on the ear directly.

As someone who’s been dealing with a concussion for 3 weeks - I completely sympathise with Smith there. Anecdotal experience suggests there’s very little chance he plays in the next test, or maybe even the one after that.

Medium risk, high reward.  If they do it they’re 2 up with three to play. They’ll likely be able to hold on for a draw if they do lose a couple of early wickets, and even if they lose it’s probable that they’ll only need to win 1 more match to retain the Ashes as it’s a distinct possibility that the weather will produce a draw between now and mid September.
